The SSM Hospice and Home Care Foundation collects and distributes funds for indigent patients who are seriously ill and dying (and their families).

SSM Hospice and Home Care (www.ssmhomecare.com) is a member of SSM Health Care (www.ssmhc.com), the first Health Care Organization to be awarded the Malcolm Baldrige National Quality Award. With SSM Home Care, the Foundation provides care in Illinois, Missouri and Oklahoma.

Patients are being served in:

• Blue Island, Illinois aligned with SSM St. Francis Hospital
• Mid-Missouri aligned with SSM St. Mary's Health Center
• Northwest Missouri aligned with SSM St. Francis Hospital
• Oklahoma City, Oklahoma aligned with St. Anthony Hospital
• St. Louis, Missouri aligned with
• SSM Cardinal Glennon Children’s Medical Center,
• SSM DePaul Health Center,
• SSM St. Joseph Hospitals in Kirkwood, O’Fallon, St. Charles and Wentzville, and
• SSM St. Mary’s Health Center

The SSM Hospice and Home Care Foundation is non-profit, 501(c)(3). Under direction of regional Boards of Trustees, the Foundation raises funds via appeals, personal and corporate donations, events and grants. Patients or primary care givers may request assistance by contacting their SSM Social Worker.
