The SSM Hospice and Home Care Foundation exists to help our patients maintain peace, dignity and comfort at the end of life. Not only do we provide a financial safety net for our patients and families through direct cash assistance for necessities such as rent, utility payments, prescriptions and therapies not covered by Medicaid, we provide an emotional safety net as well. Our newest programs enable our patients to preserve precious memories and help our littlest family members grieve.

o   MAGIC: SSM Hospice’s MAGIC (Mending a Heart; Grief in Children) Bereavement Program is designed to help children work through their grief after experiencing the loss of a loved one. This program is supported by the Foundation and includes a one-day grief retreat.

o   SSM My Legacy: through this program, our Hospice patients have the opportunity to preserve the stories, values, ideals and experiences through “Life Reviews.” These video recorded interviews are edited and preserved on DVDs that will bring comfort and historic value to their families for years to come. A photography session is additional offering of the SSM My Legacy program. Final photos can be as large as a full family portrait or as simple as a close up of loved ones hands. Families are given a full set of photo proofs, a CD with digital files for reproduction, and a framed 8x10 photo. As with MAGIC, SSM My Legacy services are available to patients and families and provided by the Foundation free of charge.
