SSM Hospice and Home Care Foundation is dedicated to providing financial, physical and emotional support to our patients and their families through our life-honoring programs.
Foundation programs offered include:
- Patient and Family Emergency Assistance: provides non-healthcare support to qualified patients and families in financial crisis, including assistance with rent or mortgage payments, utility payments, medical equipment and more
MAGIC (Mending a Heart, Grief in Children): Children’s Bereavement Program
SSM My Legacy: provides patients with life review in collaboration with the University of Missouri – St. Louis; completed life review acts as a piece of living history for the patient’s family
Comfort Touch Massage Therapy: provides hospice patients with comfort and pain-relief
Memories That Last: grants wishes for elderly or terminally ill patients
Rehabilitation: provides funding for services not supported by government or commercial pay for physical therapy, occupational therapy and speech therapy
The SSM Hospice and Home Care Foundation is non-profit, 501(c)(3). Under direction of regional Boards of Trustees, the Foundation raises funds via appeals, personal and corporate donations, events and grants. Patients or primary care givers may request assistance by contacting their SSM Social Worker.
