SSM Hospice and Home Care Foundation is committed to providing compassionate care to patients, regardless of their financial circumstances. The organization supports many Home Care and Hospice programs that enable individuals to maintain a quality of life while living in their home. The Foundaton is a member of SSM Health Care, the first Health Care Organization to be awarded the Malcolm Baldrige National Quality Award.
Foundation programs offered include:
- “Comfort Touch” Massage Therapy. Initiated in 2010, this program was created to be a gift to hospice patients in an effort to reduce the pain, anxiety and concerns that he/she might be experiencing. SSM Hospice & Home Care Foundation believes that the gift of human touch is very powerful should be shared with our hospice patients.
- Financial assistance for qualified candidates needing medical equipment, additional support or help with expenses to maintain quality of life.
- Physical therapy, occupational therapy and speech therapy programs to assist in recovery when insurance is not available.
- “Memories that Last”, a new program that grants wishes to the elderly or terminally ill.
The SSM Hospice and Home Care Foundation is non-profit, 501(c)(3). Under direction of regional Boards of Trustees, the Foundation raises funds via appeals, personal and corporate donations, events and grants. Patients or primary care givers may request assistance by contacting their SSM Social Worker.
